The Atlantis Charter School nurses are essential and valued members of our educational team. They provide students with accessible health services while promoting lifelong healthy habits. With specialized training in health assessment and wellness, our school nurses skillfully address the evolving healthcare needs of our students and ensure they receive the support and referrals necessary for their well-being and success in the classroom.
ALL MEDICATION ORDERS MUST BE RENEWED AT THE BEGINNING OF EACH SCHOOL YEAR BY A LICENSED PRESCRIBER.
At Atlantis Charter School, we encourage medications to be administered at home whenever possible. However, if a student requires medication during the school day, state regulations must be followed. The following forms (see links below) must be on file in your child’s health record before any medication can be administered at school.
- Parent/Guardian Consent: A signed consent form is required before medication can be given. Please complete and return this form to the school nurse.
- Medication Order Form: This form must be completed by your child’s licensed healthcare provider (physician, nurse practitioner, etc.) and returned to the school nurse. Medication orders must be renewed at the beginning of each academic year and as needed.
For students who need to carry an inhaler, a specific written order from the physician is required.
Medication Drop-Off & Storage
Medications must be brought to the school nurse by a parent/guardian or designated responsible adult.
All medications must be in a sealed pharmacy-labeled container as dispensed by the pharmacy or in the original manufacturer-labeled and sealed packaging for over-the-counter medications.
We recommend requesting separate medication bottles from your pharmacy for home and school use.
A maximum of a 30-day supply of medication should be sent to school at one time.
Over-the-Counter Medications
Over-the-counter medications, including Tylenol and ibuprofen, cannot be administered without a completed and signed medication authorization form.
End-of-Year Medication Pick-Up
All medications—including inhalers, spacers, nebulizers, and medical equipment—must be picked up by a parent or guardian before the last day of school. Any medication left after that time will be properly disposed of.

Authorization for Prescription Medication
The Authorization for Prescription Medication Form must be completed and submitted for any student requiring prescription medication during school hours. This form ensures that medication is administered safely and in accordance with medical instructions. A licensed healthcare provider and a parent/guardian must complete and sign the form before medication can be given at school. Please return the completed form to the school nurse.
Absence-Notification
On the day a student is absent, it is the responsibility of a parent or guardian to telephone your child’s school within the first 90 minutes of the school day to give a valid reason for the absence.
